To secure your spot on the mission trip please fill out the form below. You will also have to send in a $200 registration fee. You can either send it in the mail to Heather Black at 114 S California Ave. Morris, MN 56267 or hand it in to the office manager at your church. Please write checks out to "Fusion Ministry". If giving cash, please make sure your name and mission trip are written on the envelope.
